Evacuation Assistance Program

On-Line Evacuation Assistance Form

Filling out and submitting the Evacuation Assistance Form on-line does not mean you are registered. Once your assistance form is received, it must be reviewed for medical needs, transportation needs, and accuracy. Assistance forms with errors or missing information will delay the registration process.

Once the review of your Assistance Form is complete, if you need our assistance you will be entered into our database, and a letter will be sent to the mailing address you provided notifying you of your registration.

Do not wait for an approaching hurricane to register. Registration can be done throughout the year, and the best time to register is before hurricane season begins. If you need assistance, make this an important part of your plan.
